Survey accessibility (a comparison)

Hi everyone.
Thanks again for those of you who responded re: accessible survey platforms.

Based on responses, we’ve created a test survey on two different platforms, and would greatly appreciate any feedback re: which is more accessible.  The questions and layout should be the same so it should be a 1:1 comparison.

If it’s possible and you have the time/inclination, would you please visit them both and let us know which platform/survey works better and/or if there are any showstoppers that make them inaccessible?
